In order to make valid application for admission to Hawthorne Christian Academy the following items must be completed and submitted:
One (1) student application for each student seeking admission ($100 application fee per applicant).
Copy of student's original birth certificate.
Written copy of the student's immunization record signed by a physician or by the nurse of the student's previous school
Copy of the student's report cards, including the most current grades, and a minimum of two previous years' standardized test results and school records
Sixth through twelfth grade applicants are required to complete this form
HCA requires all students entering the school be examined by the family physician before the school year begins.
Upon receipt of the information detailed above, you will be notified of a date for scholastic testing and an interview with the Director of Admissions.
