Admissions Process

In order to make valid application for admissions to Hawthorne Christian Academy the following items must be completed and submitted.

 Form / Item  Description
 Student Application  One (1) student application for each student seeking admission
 Application Fee  One (1) $100 non-refundable application fee for each student seeking  admission.
 Birth Certificate  Copy of student's original birth certificate
 Social Security Number  Copy of student's social security card
 Immunization Record  Written copy of the student's immunization record signed by a physician
 or by the nurse of the student's previous school
 Report Cards / Test Scores  Copy of the student's report cards, including the most current grades,
 and a minimum of two previous years' standardized test results and
 school records.
 Reference Forms  Teacher Reference: completed by a current teacher of the applicant
 General Reference: completed by a non-relative of the applicant
 Church Reference: completed by a pastor, youth director, or Sunday
 School teacher of the applicant.
 * Kindergarten applicants need two references; All other grades three.
 ** Home schoolers may submit an additional General Reference Form in      place of the Teacher Reference Form.
 Biographical Sketch  Biographical sketch is required for students entering grades 6-12
 Physical Form  HCA requires all students entering the school be examined by the family
 physician before the school year begins.

Upon receipt of the information detailed above, you will be notified of a date for scholastic testing and an interview with the Director of Admissions.
